According to onchain analyst ZachXBT, a Cardano user suffered a massive loss of approximately 605 million USD worth of ADA (about 14.4 million ADA) during a routine stablecoin swap on the Minswap decentralized exchange (DEX). The Incident: A Dormant Wallet Reawakens and a Trade Goes Wrong

A Cardano wallet that had been dormant since 2020 suddenly became active. The wallet held 14.4 million ADA, worth roughly 6.9 million USD at the time of the transaction. The user attempted to swap these ADA tokens for USDA, a stablecoin native to the Cardano ecosystem, on the Minswap DEX. However, the ADA-USDA liquidity pool contained only about 1.5 million USD in total liquidity, far too shallow to absorb such a large order. When the swap was executed, the massive trade caused extreme slippage, briefly driving the price of USDA from its intended peg of 1 USD to an eye-popping 60 to 70 USD per coin.
